The purpose of this book specifically is to teach surgeons (academic or community), surgical fellows and surgical residents regardless of the surgical specialty, the skills to appraise what they read in the surgical literature. Surgeons need to be able to understand what they read before applying the conclusions of a surgical article to their practice. As most surgeons do not have the extra training in health research methodology, understanding how the research was done, how to interpret the results and finally deciding to apply them to the patient level is indeed a difficult task. Chapters explain the methodological issues pertaining to the various study designs reported in the surgical literature. Most chapters begin with a clinical scenario with uncertain course of action with which most surgeons are struggling. Readers are taught how to search the literature for the best evidence that will answer the surgical problem under discussion. An identified article that seems relevant to the problem you are investigating can be appraised by addressing 3 key questions: 1). Is the study I am reading valid? 2). What are the results of this study? 3). Can I apply these results to my patients? While the primary goal of Evidence-Based Surgery is to teach surgeons how to appraise the surgical literature, an added benefit is that the concepts explained here may help research-minded surgeons produce higher quality research.

Much anticipated, the Second Edition of Surgery: Basic Science and Clinical Evidence features fully revised and updated information on the evidence-based practice of surgery, including significant new sections on trauma and critical care and the often challenging surgical care of unique populations, including elderly, pediatric, immunocompromised, and obese patients as well as timely new chapters on the pre- and post-operative care of the cardiac surgery patient, intestinal transplantation, surgical infections, the fundamentals of cancer genetics and proteomics. Also new to this edition are discussions of electrosurgical instruments, robotics, imaging modalities, and other emerging technologies influencing the modern practice of surgery. Clinically focused sections in gastrointestinal, vascular, cardiothoracic, transplant, and cancer surgery enable the surgeon to make decisions based upon the most relevant data in modern surgical practice. The text is enhanced by more than 1,000 illustrations and hundreds of the signature evidence-based tables that made the first edition of SURGERY an instant classic.

A guide to "when to" and "what to" rather than "how to", this book provides evidence-based surgical reviews to provide credible answers to age-old surgical management questions. The management issues presented are oriented toward interventions and use evidence-based techniques to assess the safety and efficacy of new treatments and rehabilitative or preventative interventions. Each chapter is organised around the key questions essential to delineating the current status of evidence related to the subject reviewed. Publications from the past decade are cited that provide Level I and II evidence using the Oxford scale. Throughout Elective General Surgery, careful assessment of the validity of intervention studies and the strength of the evidence that they provide underlies the choices of cited publications. The information presented in this volume guides the scientific surgeon in providing state-of-the-art care and in optimizing the use of medical resources without losing sight of the need to address the unique needs of individual patients.

Shared decision making is integral to future surgical care. It is not possible without the organization and applications of knowledge. Evidence-based surgical practice is inevitable as practice and outcomes become patient centered. This issue will offer articles on developing skills for evidence-based surgery, the nature of surgical evidence, producing the evidence, and evidence-based decision making.

As the healthcare environment changes, the need for outcomes-based tre atment planning becomes even more critical. This book guides the reade r through current outcomes-based research as it pertains to surgery. F irst, it gives a complete overview of the practice of evidence-based s urgery (EBS), with topics such as treatment planning, policy issues, a nd ethical issues. Then it gives practical, step-by-step advice on the methodology of EBS, with chapters on study design, outcomes measures, adjustments for complications and comorbidities, cost, and data sourc es. Last, it publishes the results of numerous respected EBS studies.

This text provides a comprehensive and state-of-the-art overview of the major issues specific to the surgical decision-making process. These include patient’s anatomy and pathophysiology as well as the magnitude of the injury at hand, the surgeon's own physiologic and mental status, training and experience, and many other factors such as creativity, leadership skills, and overall "biochemistry" of the environment. The text reviews theoretical as well as objective information that surgeons use to make intraoperative decisions in situations, often with very limited data; decisions that will decide between a patient's living or dying, such as in trauma surgery and other complex surgeries. How surgeons choose one technical approach over another in these situations is covered. This book fills a critical need for resource materials on these topics and includes both theoretical as well as practical presentations of many typical patients seen in operating rooms around the world. Surgical Decision Making: Beyond the Evidence Based Surgery is written by academic and clinical practicing surgeons that face intraoperative decision situations on a daily basis and therefore provides a unique and valuable resource in the field for surgeons currently in training and for those already in clinical or research practice.

Evidence-based plastic surgery integrates the best research evidence with clinical expertise and patient values. The purpose of this issue is to assist surgeons at any stage in their careers, from residents and clinical fellows to established clinical surgeons and academic practitioners, to address important clinical questions through correct research methods. Topics covered in the issue include how to formulate clinically important questions; what study design can best answer a specific research question; how to measure the results of surgical interventions, in particular the health status and quality of life of patients; and how to evaluate one's practice through cost-effectiveness analysis.

In this textbook, leading experts from highly acclaimed institutions describe evidence-based best practice in the management of a wide range of benign and malignant thyroid, parathyroid, adrenal, and neuroendocrine conditions. Detailed attention is devoted to the current role of surgery, including minimally invasive surgery and robotic surgery, in different endocrine disorders. The reader will also learn how best to respond to the problems that may be encountered during endocrine surgical practice. While much of the focus is on surgical aspects, the approach is multidisciplinary, with inclusion of information on recent advances in epidemiology, genetics, cytology, pathology, imaging modalities, and other treatment options. The clear text is complemented by instructive clinical cases as well as numerous high-quality illustrations and tables summarizing key points. This book will be of value for specialists in endocrine medicine and surgery as well as general surgeons with an interest in endocrine surgery.
